Care Coordinator (Non-clinical)

 

Point of Pride is a 501(c)(3) non-profit organization that serves the international transgender community through direct service, grantmaking, and other life-saving programs. From 2016-2022, we were volunteer-operated by a trans-led Board of Directors. Over the last few years, Point of Pride has grown exponentially and is staffing a team to reflect the current size—and future ambitions—of our work.  Now, we are seeking a full-time staff member to join our team: a care coordinator.

The care coordinator is a non-clinical role that reports to our Director of Programs and empowers the people we serve (both on an individual level and on a broader community level) in navigating their healthcare journey successfully. The position is a mix of case management, content development, and day-to-day administrative support.

In this role, you’ll get to:

  • Support recipients in meeting their goals. You’ll work closely with financial aid recipients to develop a comprehensive plan that identifies their unique challenges, establishes a timeline of tasks, and helps them navigate all steps to utilizing their grant award. You’ll refer recipients to outside resources and organizations for additional support as needed. If applicable, you’ll serve as an advocate to help recipients resolve challenges with health insurance, their providers’ offices, and other challenges.
  • Communicate with applicants and members of our community. You’ll be the first point of contact for people who are seeking support for themselves or a trans loved one, helping to connect them with services or third-party support resources as needed.
  • Help share recipient stories to foster trans joy. You’ll identify recipients who are willing and eager to share their journeys with the broader community. You'll support other members of the team in capturing their stories, providing visibility to the issues our community faces and transparency to the work we do.
  • Collaborate in developing educational content for the people we serve as well as the broader community. You’ll identify areas of opportunity for Point of Pride to better serve our grant recipients and applicants based on your interactions and assessment. You’ll collaborate with other staff and external partners to produce ongoing blogs, resource guides, health and wellness materials, and live/recorded info sessions that provide valuable information related to gender-affirming care and the trans experience. 
  • Support administrative needs. You’ll work alongside the Director of Programs to ensure payments to our external partners and service providers for recipient healthcare needs are timely, accurate, and properly documented. You’ll also oversee all follow-up and exit interviews with the people we serve to ensure we deliver exceptional support and identify gaps or growth opportunities.

We are looking for people who:

  • Possess excellent, empathetic, culturally-competent communication skills
  • Possess a broad holistic perspective of healthcare needs and social determinants of health, while maintaining attention to detail on an individual’s unique or complex needs
  • Have an understanding of diverse groups of people, especially issues and topics affecting trans youth, elders, unhoused people, and people living with a disability or chronic illness
  • Maintain accurate, up-to-date records that foster clear, effective communication between multiple stakeholders 

Our ideal candidate:

  • Has previous experience in care coordination, case management, social work, healthcare administration, or a related field
  • Is knowledgeable of healthcare systems, insurance, and community resources
  • Is experienced using Google Workspace (Gmail, Docs, Sheets, Forms)
  • Is familiar with project management tools or software – knowledge of Basecamp a plus
  • Is tech savvy and enjoys learning new technologies – knowledge of HubSpot a plus
