Right now, your voice matters—especially in this moment.

Health care for trans communities in the U.S. faces unprecedented challenges and risks. Because of what we are going through as a community, this survey is more important than ever: it’s the most up-to-date, community-led effort in 2025 to listen directly to the voices of transgender and non-binary people. There is no other survey capturing our experiences and needs like this one—right here, right now.

Your experiences shape change. By sharing honestly, you help build more affirming, accessible care for all of us. Some questions may feel personal—skip what you need, go at your own pace, and know your responses are confidential.

This project is led by Point of Pride, a trans-led nonprofit committed to expanding access to life-saving healthcare. The survey was shaped by a diverse steering committee of trans and non-binary individuals with a wide range of lived experiences and professional backgrounds.

What you can expect

This survey is made up of several sections, each with a short description so you know what’s coming. You can take your time as you move through—it should take about 25-35 minutes to complete, depending on how much you choose to share.

For your privacy and safety, this survey is anonymous and we do not collect any identifying information. Because of this, you won’t be able to return to your survey later once it’s started. This helps us ensure your responses stay fully confidential and secure.

Protecting your privacy and security is incredibly important to us.

  • Your responses are confidential and completely anonymous. We will never share your data with anyone outside our trusted team.
  • We do not collect any personally identifiable information such as your name, email address, mailing address, or phone number. This means your answers cannot be linked back to you.
  • This survey is hosted on JotForm, a secure platform that uses industry-leading 256-bit SSL encryption—the same level of protection used by online banking services—to keep your data safe from unauthorized access. Every form is individually encrypted with a unique password, adding an additional layer of airtight security.
  • At the very end of the survey, you'll be asked whether we can anonymously quote any information you share in any of the optional short-answer responses. This is completely optional and designed to help us share your powerful stories while protecting your privacy.

At the end of the survey, you’ll be redirected to a webpage filled with caring resources designed to support and uplift you. We’ve included trusted hotlines, trans-affirming meditations, a vetted LGBTQ Healthcare Directory and a variety of community connections—because no one should have to navigate their journey alone. We hope these resources bring you comfort, guidance, and a reminder that support is always within reach.
